REGULATIONS ON UNIVERSITY-SPECIFIC COURSES

ANNEX V

ISSUING OF UPV/EHU-SPECIFIC FINAL QUALIFICATION CERTIFICATES & OTHER ACADEMIC CERTIFICATES

CHAPTER I. ISSUING OF FINAL QUALIFICATION CERTIFICATES AND DIPLOMAS 

Article 1.- General Rules

Final qualification certificates are awarded by the Rector in a standardised format, and entered in the University Register of UPV/EHU-Specific Qualifications. The requirements for identification, safeguarding, certification and public status are similar to those for the University Register of Official Qualifications.

Article 2.- Procedure

1.- Request for issuance

1.1- Requests for the issue of final qualification certificates must be submitted to the UPV/EHU-Specific Qualifications Unit and the office of the Vice-Rectors for the Gipuzkoa Campus, the Bizkaia Campus and the Álava Campus, accompanied by the following documents:

a)  Application by the interested party.

b) (Photocopy of national ID card (for Spanish nationals), official identity document (citizens of other EU countries) or photocopy of passport or residence card (non EU citizens).

c)  Receipt proving payment of the relevant fees. Persons deemed exempt must proceed as per Article 8 of Annex I.

The UPV/EHU-Specific Qualifications Unit will issue a certificate attesting to payment of the fee for the issue of the relevant final qualification certificate or diploma, which will serve provisionally in the stead of the final certificate for all relevant purposes until the latter is issued. The said certificate will be issued in a standard format.

When final qualification certificates or diplomas are obtained thanks to recognition of other qualifications and independent modules, this must be duly indicated on the reverse side of the certificate.

1.2 - Along with the documents indicated in the previous point, persons who took UPV/EHU-specific qualifications or diploma courses prior to academic year 93-94 must submit the following:

1.  A legalised photocopy (or a plain photocopy plus the original for comparison) of the official degree certificate that entitles the holder to access the UPV/EHU-specific qualification, or a receipt proving that it has been applied for. This document is not required for qualifications entitling the holder to access specific qualification courses which were obtained at the UPV/EHU as from 1988.

2.- Handover of final qualification certificates and diplomas

Once final qualification certificates or diplomas are printed, they must be collected in person by the interested party from the UPV/EHU-Specific Qualifications Unit. If interested parties are unable to collect their certificate, they may authorise someone else by means of a notarial power of attorney to collect it on their behalf.

Notwithstanding the foregoing, interested parties who reside outside the location where the UPV/EHU-Specific Qualifications Unit is located may submit a written request to the said unit for their final qualification certificate to be sent to the Office of the Vice-Rector of the Bizkaia Campus, the Office of the Vice Rector of the Gipuzkoa Campus, the Office of the Vice Rector of the Alava Campus, the Provincial Directorate for Education and Culture, the Education Office or the consular office nearest to their place of residence.

In exceptional circumstances, in those provinces where it is not possible to hand over certificates or diplomas via ministerial offices, they may be sent by registered mail to the student, who assumes all risks of potential damage to the document and agrees to pay any resulting costs for the issue of a duplicate.

The procedure set out for collecting final qualification certificates will also apply to the collection of duplicates, the issuing of which is regulated as per subsection 4 of this article.

Should the final qualification certificate not be collected within five years as from its issue, the UPV/EHU-Specific Qualifications Unit will draw up a resolution which will be published in the Official Gazette of the Basque Country giving a further period of one month for its collection and announcing that if it is not collected within that time the final qualification certificate will be cancelled and destroyed. Any destruction of a final qualification certificate or diploma will be recorded in an official record and noted down in the University Register of UPV/EHU-specific Qualifications.

The Rector of the university will rule on whether it is appropriate to hand over to next of kin on request any certificates issued to persons who have completed the relevant studies but passed away before collecting their certificates. Such certificates will bear the relevant procedural note printed at the bottom of the reverse side.

3.- Adaptation of the wording of final qualification certificates to the gender of their holders.

Names and other wording contained in final qualification certificates must be expressed so as to reflect the gender of their holders.

4.- Issuing of duplicates

a) Issuing of duplicates due to damage or correction of errors:

The correction of any errors in the contents of a qualification certificate, and any deterioration of certificates not attributable to the University will be grounds for the issuing of a duplicate on request from the interested party. The original certificate or that part thereof which permits the original certificate to be identified must be handed in together with the documents evidencing the reason for requesting a duplicate.

The fees payable in such cases will be for the account of the interested party.

For the correction of any errors attributable to the University, the UPV/EHU-Specific Qualifications Unit will commence the procedure for the issuing of a duplicate ex officio, and the interested party will not be required to pay any fee for same.

The procedure for the issuing of duplicates must be commenced at the UPV/EHU-Specific Qualifications Unit.

b) Issuing of duplicates due to loss or theft:

For the issuing of duplicates of final qualification certificates for UPV/EHU-specific qualifications when the originals are lost or stolen, a formal application from the interested party is required, indicating why a duplicate is requested.

If the loss or theft of a certificate is attributable to the University, the UPV/EHU-Specific Qualifications Unit will commence the procedure for the issuing of a duplicate ex officio, and the interested party will not be required to pay any fee for same.

The reason for the issuing of duplicates must be printed on them as a procedural note at the foot of the front face.

CHAPTER II: ISSUING OF CERTIFICATES

Article 3.- Requesting of Certificates

1.- All students who complete the studies for a UPV/EHU-specific qualification with an overall PASS grade may request a certificate to that effect from the UPV/EHU-Specific Qualifications Unit and the offices of the Vice-Rectors of the Bizkaia, Gipuzkoa and Álava campuses. To that end they must provide the documents indicated below (for students enrolled from academic year 93-94 onwards).

a) Duly completed application form.

c)  Receipt proving payment of the relevant fees.

2.- Students enrolled in academic years prior to 1993-94 must file requests for the issuing of academic certificates at the Office of the Registrar at the centre, department or institute where they were enrolled, and must provide such documents as may be requested by same.

3.- Students who fail to obtain an overall PASS grade may request a certificate of attendance. Such certificates will be signed by the Academic Committee, which will also set the conditions for their issue

CHAPTER III. ARTICLE 26.- FEES FOR THE ISSUING OF FINAL QUALIFICATION & OTHER CERTIFICATES

The fees for the issuing of final qualification certificates for UPV/EHU-specific qualifications and other certificates are as set by the Social Council of the UPV/EHU at the proposal of the relevant committee, and will be published by the office of the relevant Vice Rector on the UPV/EHU website.
